HOW TO CORRECT DEEP HIGH-FOLD CREASE
Some people have deep high-fold crease issues after having eyelid surgery. It is a very frequent and tough-to-solve problem. This issue is common because of a misunderstanding of the appropriate principles for double eyelid surgery.
This misunderstanding arises from both those undergoing surgery who often need large and high eyelids without knowing their own anatomical limitations. Another part is that doctors also do not understand the principle of double eyelid surgery. The subsequent problem of making the eyelids too high, in addition to looking unattractive, some people may have eyelids that look swollen, no matter how long they wait, they will have eyes that can’t fully open, sunken eyes, look tired, and look older. Understanding the high eyelid problem’s solution is challenging. Some people have two scars—one from a previous wound and one from the same wound. Some individuals just have one scar.



There are three factors to consider while correcting high eyelids.
✅ The appropriate eyelid height for eyelid suture should be the appropriate height for each person look according to the height of the eyelid cartilage or tarsal plate of that person because the appropriate point is at the upper edge of the eyelid cartilage.
✅ The scar or anchor point from earlier double eyelid surgery is too high to be placed at the new eyelid level; it is too far from the incision line.
✅ The existing eyelids are more or less slack depending on the excess eyelids of each person.
The distance between the previous scar line and the new wound line is measured and checked to see if the eyelids are cut between these two lines, if the eyes are totally closed or not, and if the eyes are still closed, the skin can be cut. If the eye can be removed, there will be only one scar, but if it cannot be removed, there will be two scars. Once assessed, the procedure of double eyelid repair will be performed.
The three main principles are as follow:
✅ Excision of the original eyelid fixation line.
✅ Creation of a new lower eyelid adhesion line.
✅ Prevention of reattachment of the strabismus.
